Disability

Paradigm Psychiatry requires a patient to be established with the practice for a minimum of 90 days before any disability requests can be completed.

A $75.00 Administration Fee is charged for completion of all forms, documents, or letters including FMLA, Short Term Disability or ADA (Americans with Disabilities Act).

A $25.00 fee will be charged for all ESA (Emotional Support Animal) letters.

If requesting a leave of absence or disability, an appointment is required with your provider to evaluate capabilities and limitations, in addition to regular scheduled follow up appointments determined by your provider.

Paradigm Psychiatry does NOT complete Permanent or Long Term Disability Requests.
