Payment for all services rendered is required in full at the time of your visit. This includes any estimated insurance responsibilities such as deductibles, coinsurance percentages, copays, self-pay rates, outstanding balances, fees, and any other amounts due for services or additional treatments provided. Paradigm Psychiatry accepts most credit cards, debit cards, HSA/FSA cards and cash. Personal Checks and Care Credit not accepted.

Self-Pay:

Paradigm Psychiatry offers a self-pay option for patients who either do not have insurance coverage, have an Out-of-Network insurance, or have an insurance plan not accepted or contracted with Paradigm Psychiatry.

Paradigm Psychiatry Self-Pay Rates:

$250.00 - New Patient Intake (60 minutes)

$150.00 - Follow-Up Visit (30 minutes)

Credit Card on File Policy:

To streamline the payment process and ensure timely payment for services, all patients are required to sign a Financial Agreement Policy and keep a saved card on file with our office. This card will be used to cover any charges including deductibles, coinsurances, copayments, self-pay rates, balances, and any other fee due.

Automatic Charges: Your card will be automatically charged at the time of service before your appointment and for any balances that remain unpaid after an invoice has been provided. It is the patient's responsibility to notify us if the card on file is no longer active or if there are any changes to your payment method. Please note, Paradigm Psychiatry is not responsible for any overdraft or other fees that may result from processing your payment.

Invoices:

Outstanding balances will be sent as electronic statements via email, text message, and/or through the patient portal account. Each invoice will include a secure payment link for easy online payment. Pay by phone is also an option by contacting our office main line. Payment is due within 7 days of receiving the invoice. The saved card on file will be automatically charged to collect a balance if unpaid after 7 days. Outstanding balances are expected to be paid prior to the next service being provided. Paradigm Psychiatry reserves the right to temporarily pause all future services if an outstanding balance has not been resolved or if an account becomes past due 30 days or more as described in our contract terms.
