Fees
Self-Pay
Self-pay rates are available for uninsured or out-of-network patients.
- $250 - New Patient Intake (60 minutes)
- $150 - Follow-Up Visit (30 minutes)

Payment
Copays, deductibles, coinsurance, and other estimated patient responsibilities are due at the time of service.
