Before Your Appointment
All required forms, psychiatric screening assessments, consent forms, and current insurance information must be submitted through the Patient Portal at least 48 hours before your scheduled appointment. This allows your provider time to review your information and our office to verify your insurance eligibility and benefits. Appointments will be cancelled if the required information is not received at least 48 hours before the scheduled appointment and may be offered to another patient on our waiting list.
If available, please provide a current medication list and any relevant medical or psychiatric records, psychological testing, or previous evaluations.
Please arrive 15 minutes early for your first appointment to allow time for check-in. Patients arriving more than 10 minutes late may need to reschedule, and a $75 fee may apply.
For Minors
A parent or legal guardian must complete the required forms and provide consent before the initial appointment. Please provide relevant school records, IEP/504 plans, psychological testing, or previous evaluations, if available.
