New Patients
- Email your insurance card (front & back) and photo ID (front & back) to info@paradigmpsychiatry.com. If you're not the subscriber, include the subscriber's full name, date of birth, and relationship to you.
- Check your email for a patient portal invitation and complete all documents under Questionnaires at least 48 hours before your appointment.
All Patients
- Arrive 15 minutes early and bring your insurance card and photo ID.
- Bring any relevant medical records (psychiatric history, lab results, hospital discharge summaries, etc.).
